Don't wrap yourself around the axle about the parental consent form. It won't be an issue for the t-shirt, because I will be getting the players' mailing addresses from the team captain. If you want a shirt, give your address to your team captain, if you don't, don't give him your address. For the larger prizes, if there is an issue with age, we'll clear it up at that time if need be with a consent form, either by way of mail, fax, however, but it shouldn't be a problem.
Again, if in doubt, go ahead and sign up. I'd rather have you signed up and play and have to deal with figuring out how to give you the prize you win than not have you signed up at all, so if in doubt, go ahead and sign up. We'll deal with prizes if you win.
