Mass Effect owes me like 2 achievements. Even my friends have had this problem where we went through the game a certain way just to get the damn achievement, and then didn't get it in the end. I know one of them was having a human teammate with you 5% of the time or something. Whatever.

Are you talking about the Ally achievements? If so, you need either Ashley or Kaidan on your team while completing several assignments and nearly all the missions.

I honestly have no clue what you mean by having a human on your team 5% of the time. No achievement puts it that way. =P

If this is what you think, then that's exactly why you didn't unlock the achievement... you need to complete a majority of the story with a human teammate to get that certain achievement.

Between you and me, I had Wrex and Garrus in my party for a majority of the game, yet I didn't get either one of those achievements... not that I am bitter or anything.

One guide suggests that you need to complete 45 assignments and 15 missions, but I've found this to be somewhat wrong, when I received Quarian Ally. I had started somewhere around 45 assignments, but did not complete them all. As for missions, I think I may have had 12-13 done when I received it.

It doesn't necessarily mean that you play from Point A to Point B with those characters, but complete all the stuff in between as well. It's still a little strange to me, since I got Quarian Ally at an unexpected time. It may even actually be somewhat dependent of the character, considering all the things I completed with Tali I had done so with Liara, yet I never received the Asari Ally achievement. I'm also wondering if these completion rates carry over to new games started with the same main character.

I received my Human ally and Quarian ally at the same time and both of those achievements came about 10-15 minutes AFTER I received the Completionist achievement or whatever completing majority of the game is called.

The Completionist achievement is basically like your own character's achievement for completing the majority of the game, so it's natural that you should receive other Ally achievements at or around the same time, assuming you used those allies from beginning to "end."

Asari Ally seems to be the hardest. I just don't understand why I unlocked Quarian Ally but not Asari Ally (on one playthrough) when I did everything with them. I ignored all assignments and missions -- missions you can do with Liara -- until I got the asari. I had first thought that it was because I locked one mission: I started and completed the Smuggling assignment before doing anything else on Noveria, and I went with turning in Opold, the smuggler, so that I could receive the Peak 15 pass from Anoleis. This, in turn, did not allow me to complete the mission "Lorik Qui'in" and locked it in my list as incomplete for the rest of the playthrough. This is my only guess as to why I did not receive it, but I did do a few more missions after I received Quarian Ally and still did not get the achievement (I think I got the achievement on an assignment).

In any case, some of these achievements don't seem to be consistent. =P
