One of my all time favs is Disturbed's cover of Fade To Black by Metallica. They did it live at the Music As A Weapon tour a few years back. David Draiman's voice just seems to suit the song so perfectly.

My favourite cover is certainly Cradle of Filth with Hallowed Be Thy Name (Iron Maiden).

Honourable mentions go to:

The Kovenant - The Memory Remains (Metallica)
The Kovenant - Spaceman (Babylon Zoo)
Vader - Raining Blood (Slayer)
Graveworm - Losing My Religion (REM)
Monster Magnet - There's No Way Out Of Here (David Gilmour)
Type O Negative - Summer Breeze (The Isley Brothers)
[spunge] - No Woman No Cry (Bob Marley)
Fear Factory - School (Nirvana)

If nobody's heard of Ten Masked Men, visit Youtube for some top notch covers.

I totally forgot, but I have to add:

Spiderbait - Black Betty

I found this version the other day, and I really really like it.

A link to the video on youtube. YouTube - Spiderbait - Black Betty

With or without the vacuum solo? Gotta love that Fishman cloning Prince. HA

Off the top of my head I'll say SRV's Little Wing is pretty up there.

As for Phish covers, Frankenstein (Edgar Winter) and Also Sprach Zarathrustra (2001 Space Odyssey theme) always managed to make the show a bit more smoky!

Hearing Primus go from " South Bound Pachyderm " into Pink Floyd's " In The Flesh " in its entirety and back into SBP Live was one of the greatest moments in my life.

Ministry covering Bob Dylan's " Lay Lady Lay " has also been one of my Favs.

That's because no one can top Cobain.
