I hit level 19 last night and got the Mysterious Stranger perk, but he hasn't showed up once yet. I haven't been in a whole bunch of combat yet but how often does he usually show up?
He'll show up a bunch of times but you do need decent luck.
By time you get to level 19, it's pretty much pointless to have him because you should be strong and decently rich with good weapons by now.

I didn't really want any other perks until I got to lvl 20 since I have pretty much everything I need/want. And yeah, my luck is really low. I got to lvl 18 and got intense training to bump my luck up to 6 just so I could get this perk. I guess that's the reason I haven't seen this mysterious stranger yet...
Looks like we'll have to wait a bit longer for the other DLC

VATS is beginning to annoy me. Guns seem to get broken way faster if you use them in VATS, and I miss about 20% of the time when it says I am almost guaranteed a hit.
There are perks that you can choose which pretty much improve your aim in VATS... also, I repair all of my weapons myself, and horde duplicate weapons from all of the people that I kill in the game.
My Chinese assault rifle and combat shotgun do mass amounts of damage now...

I have a luck of 6, Level 15. I got the perk today at level 14. He's showed up for me about 5 times now.
VATS is beginning to annoy me. Guns seem to get broken way faster if you use them in VATS, and I miss about 20% of the time when it says I am almost guaranteed a hit.
VATS gives you stronger hits and a better chance at criticals while allowing you to slow the whole game down to allow you to think and deal with multiple enemies at once if you get overwhelmed.

Same. I even throw on my engineering jumpsuit too so it raises my repair skill before I take all of my duplicate weapons to repair my main one.

The slow motion close up display of my enemies bodies being blown to bits is a healthy reason as to why I go into VATS a lot even though I'm strong enough and have awesome weapons to just run around and shoot everything from normal view and save time. VATS just makes it more fun.
Intelligence is so much better than the rest of the SPECIAL attributes... I set it to 9 right at the start, so I got 19 skillpoints per level... with a little help from a pair of perks (Comprehension to douible the value of skill books and the 3 more points per level one) I soon had six skills at 100... that increased to 8 by level twenty, and I have two other high skills. I laughed my *** off when I saw my brother's skills, because he followed my advice and "spread it around" but he was getting fewer points per level... all of his skills are over fifty but none of them are much higher than that.
When I first saw the map with the Explorer perk I thought there was a whole lot of game left to play, but many of the places are worthless... gas stations and B&B's out in the wilderness with a few raiders around but nothing special.
Is there a way to aim the bombs dropped from the satellite (through the satcom arrays)... its a nice little light show but I want to kill something.

True... but you actually do find valuable items in some of those "out in the middle of nowhere" places. Things like bobbleheads, books, Keller tapes, etc etc...
Hacking is easy when you back away from the terminal after three unsuccessful tries at hacking it... I'd also highly recommend trying the first word on the left side of the screen, then the first word on the right side of the screen if the one on the left didn't hack the terminal. If neither of those words work, then I normally try the second word on the right side of the screen... unless of course, the second word on the right has many of the same letters as the one you chose before it.
