(sigh) I'll answer this, but next time, try posting a thread in the proper forum.
When you pick up the skull, that is considered obtaining it. If that skull is an achievement, then the second you pick it up, you should obtain the achievement. I could be wrong, but i believe that if you die AFTER you grab the skull, you must pick it up again or else it wont count when you beat the level.
